Urgent Help Needed: Ada Letter Legally Sufficient For My Employer?

Hi All, I have AuDHD (high functioning autism with combined type ADHD). I have been working fully from home for the last 8 months after and a week ago my HR lady called and told me company policy is work from home no more than 2 days a week. I reminded her of my AuDHD and that returning to an office even 3 days a week would reel havoc on my life in many ways so I stated I was requesting accomodation to stay fully remote as my role is entirely remote in structure and function anyways and recent internal survey shows I've had a 25% increase in management satisfaction from my team since I've been remote. (I'm an admin supervisor and my entire team works in different states)

I wrote a the attached letter according to ADA, EEOC and GINA guidelines and included a statement of physician support for my therapist (Dr. of LCSW) a testing to the validity of my letter and that it provided all necessary info. I wrote the letter in lieu of the 7 page physician certification questionnaire my employer provided as it was overly broad in scope of questiontioning and felt like it was geared more towards physical disabilities along with the fact I didn't want to make my Dr. fill it all out if not necessary. I did research that the ADA doesn't require the use of an employer form.

HR is now saying they can't accept their form with see attached and the Drs certification and signature on it in conjunction with my letter (that's also signed and certified by Dr.) and that "these documents (their form) are part of a process and must be followed per our legal team".

They are asking that I now have my Dr fill out their form again and have given A deadline of next Friday.

Is this legal? Isn't the fact that I verbally disclosed my disability to my manager and HR verbally several times and wrote a compliant letter confirming my needs and life long disability signed by my Dr enough?
