Had An Odd Interaction With My Employer Tonight.

I started working at a new place somewhat recently and at the end of my shift I was approached by a manger and asked to sign a form that said I was offered the breaks that are legally required by my state (Colorado, which fromwhat i understand has pretty strict labor laws regarding breaks). I wasn’t offered any breaks for the entirely of my 8 hour shift nor the days prior and while I complied with signing it (apparently we have to sign them every night) because it was late and I just wanted to go home, it left a bad taste in my mouth. The other employee who I was closing with also signed one. I’m a bit conflicted as I just started here and don’t want to face and repercussions for potentially reporting this but at the same time I’m pretty sure it’s blatantly illegal.
