Former Employer Still Has Me On Their Website In A State I'm Not Licensed To Practice In

Hi everyone,
I am a veterinarian and recently learned that the vet I worked at before vet school still has my photo and bio up on the website's Meet the Team page. The only thing is that the bio is not just about "vet assistant DragonJouster" it is a bio for "Dr DragonJouster", with just my first name and not my last name. It also appears to have information taken from my LinkedIn resume regarding my internship and master's degree as well. It states how Dr Dragon Jouster is very much looking forward to returning to the practice.....I never signed anything orafd any promises to come back and work for her.
The vet I worked for was the definition of a crazy toxic narcisist and I spent many years in therapy during vet school dealing with the trauma she caused me. So this has brought up a bit of personal inner turmoil as well. It also makes me concerned about her sanity which I suppose is besides the point.
This vet practices in a different state than me, and I am not licensed in that state. So the big concern is that she is advertising veterinary medical services from me on her site in a state I am not legally able to practice in. It's unclear to me if this could land me in legal trouble or not with the veterinary medical board.
She is also a crazy narcissist so part of me wonders if she is doing this to be involved in my life in some freak way.
I think my first step is to call the practice, ask her to remove the photo and bio, then email and document that I called the office and made the request.
If she doesn't comply, what are next steps? Contacting the veterinary medical board? Have a lawyer send a cease and desist? Thanks for any advice. It's 1am and I can't stop thinking about this horrible woman and how she keeps coming back in to my life.
