Bit By My Friends Dog, I Don't Want Anything To Happen To It... Animal Control Requesting Information

This past weekend I was bit by my friends dog while petting him at a party. He was growling and barking his teeth but the music was loud and it was dark so I didn't notice.

I went to the doctor the next day and they made me fill out a form with details about the dog. I told the receptionist I didn't want anything to happen to the dog so she just said to "fill it out to the best of my ability". So I didn't mention my friends name, address, etc. and left everything pretty vague.

The dog has bitten someone before but he isn't really a threat to the public or anything as they live in a gated yard out in the country and don't really even have company over that often. Any company that comes over is aware to be careful with the dog. (He was abused as a puppy before they adopted him so he is just very defensive, opposed to being aggressive)

Fast forward to today, animal control left a warning at my door stating I need to contact them immediately or they may pursue further action. What are the consequences if I refuse to give them anymore information?

I would prefer to just tell them I met the owner at a park or something and didn't exchange info with them.... but if I do this I assume they will make me take a rabies treatment (the dog I was bit by is up to date on all of his shots)

Any advice would be appreciated
