A Complex Name Change Situation.
Location: Alameda County, California.
A friend of mine wishes to change her name to match the name she has used for decades. This is complicated by her name being different in different records records.
(Name is altered for privacy here.)
- The name she has been using, and wishes to standardize on, is "Nicolette Maria Cohen." (Her husband's last name is "Cohen.")
- Her birth certificate reads "Maria Malette Staub."
- Her academic records read "Nicolette Maria Staub."
- Her (US) passport reads "Maria Malette Cohen."
- Her Social Security card reads "Nicolette Cohen."
- Her California state ID card reads "Nicolette Staub Cohen."
- We're still looking for what her California marriage license has as unmarried and married names.
So… some confusion. The California NC-100 form has lines for multiple name changes, but they are pretty clearly for different natural persons (like a family changing their name).
Her goal is to have an order changing name from each other prior names to her standard name so she can get her records to line up.
As I type this, I think that "get a lawyer who specializes in vital records" is the right answer, but is there a way of doing this pro se that is not insane?
