Order For Individual Adult Change Of Name

Idrk if this is the right place for this or not but it was the only subreddit i could think of. So im filling out an order for individual adult change of name and theres multiple parts where im not sure whether i or the court fills it out. theres two sections asking for a date, one being the date in which my forms are officially petitioned in court, and one being the date in which my forms are verified by the court. i obviously dont have these dates as i havent even mailed the paperwork in so id assume thats for them to fill out? Then, at the top it says
Present: Hon. ________________________ Justice of the Supreme Court
On the back it also says

What the fuck does that mean dude????pls save the "ur an idiot" comments, ik most of this is probably common sense but im just scared ill do it wrong and have to go through this whole process again so im trying to verify from multiple sources to be safe
