My Grandmother Is Having Medical Problems And Wants To Leave Everything To Me When She Passes But I Don’t Know How To Go About This.

Hi, I (23f) just moved in with my grandmother (82f) at the beginning of May to help her maintain the house. She has been experiencing episodes of dementia but she has made it clear for the past 8 years that her house will be mine once she passes. She hasn’t updated her will in probably about a decade but I know a will is basically just a “wish list” and if I inherit her assets I will have to pay an ungodly amount in taxes. I have spoke with an Elder Law attorney and they stated that my grandmother has to be present but she doesn’t do well with traveling and she can be confused sometimes. I have heard about a “Lady Bird Deed” but I don’t know what that entails with taxes. I am looking to become her POA as well.

What is the best route to plan for this transition?

Side note : I am her only living relative and I don’t have anyone to assist me so I have come to Reddit for some help. I am also based in Florida if that means anything.
