My Boss Is Gaslighting Me And I Can't Report Him To Hr Because The Hr Rep And Him Have Been Friends For 30 Years

I work 3rd shift at a grocery store.
My job has recently hired a new non-union supervisor. In that time I have been told that my reasonable accommodations have not been submitted, I have been chronically late, and I call off constantly. I've also been told that I need to work harder because I'm falling behind, even though I have outperformed every one of my coworkers consistently.
I cannot leave this job. It was the only job willing to work with me after having my TIA (stroke) in two years. We have had verbal agreements as well as written documentation explaining all of my problems and I worked there for over a year with one small incident before the new supervisor was hired. None of that is being honored, and the written documentation has suddenly vanished.
Since he has been hired my hours have been knocked down to 16 hours a week, my time clock is magically clocking ONLY ME out 13, 14, 16 and 20 minutes late, I suddenly have this no call no show after being told not to come in, I am unreliable, unpredictable, and unprofessional on a regular basis.
I'm a social person in the workplace. I like to talk with the people I work with. Now, I have to keep to myself because if I'm "caught" talking then it'll be a writeup. I have to time my bathroom breaks because if I'm "caught" using the bathroom too long, that's a verbal warning. I have to notify my supervisor any time I have yo go do things on the computer, because if I'm "caught" doing my job, I've been missing for 45 minutes even though I've only been doing mandatory training, and I've been up there for 7 minutes. I had a panic attack in front of him and he asked me if it was because I had bad weed.
I have a panic disorder. Every panic attack feels like a heart attack. I have had the ambulance called for me in three separate incidences, all of them went unrecorded. There is nothing on our system of these ever happening, and video footage has strangely been removed to "make room on the server".
Not really sure what my options are. My store supervisor is doing nothing about this other than saying, "I haven't heard anything about this. I have no clue what the problem is. I don't know what you're talking about. This doesn't make any sense." And keeps acting like nothing is happening.
I recently reported the new supervisor to my union representative for calling me a liar, a child, and harassing me about the no call no show that never happened (which I was unable to fight) and was told that those conversations never happened, that I'm blowing things way out of proportion, and I'm behaving unprofessionally.
I am very concerned I'm about to lose my job. There are so many more of these situations that I wouldn't have enough time to type them all out.
