Granddaughter Transfers Grandmother's Car Title When She's High On Narcotics

I'll make this story as short as I can. My mother took care of my elderly (90) grandmother for many years. None of my mother's sisters were willing to take care of her. When my mother was more financially stable years ago, she even built my grandmother a house and made the $10,000 down payment on a 2015 Malibu. We moved away from my grandmother about 5 years ago, and she currently has no family living near her. 2 years ago, my grandmother told my mother that she isn't able to drive anymore, and that she wants my mother to have the car back. My mother accepted, but then my grandmother decided to keep the car to let people from her local Kingdom Hall drive her around in. She told my mother that she would still give her the car back when she no longer needed it. They talked about it multiple times, as recently as a few months ago.
Now, my grandmother is very sick. My first cousin was the only extended family member to agree to go help take care of her, while my aunt makes arrangements to move my grandmother closer so they can take care of her. All 3 of my aunts knew about the arrangement with the car. Yesterday, my aunt texted my mother and said "(cousin) took the car. They just got done transferring the title." Apparently, my grandmother decided to give my niece the car as a gift for taking care of her the past week. My grandmother is living in a state of constant high on narcotics and painkillers. Is this legal? The car is already transferred, and the niece doesn't want to give it back. What can we do about this?
