Advice Needed For Elderly Parent

At a loss and hoping for some direction as for the first steps to take. My Father In Law is not in good shape at the moment. He is dealing with alzheimer's, diabetes, dementia among other things. We feel that he is at the mid-stage dementia point as he was able to do some things for himself. Over the past two days, his condition has worsened to the point of him thinking he was on vacation while sitting on his couch, thinking Donald trump was there with him helping him 'clean out a desk', amongst other things. We tried broaching the subject with him of late life care with things such as POA, living wills, in home care etc six months ago and he dismissed it. Now I believe we are at the point where something has to be done. We live over an hour away, work 55+ hours a week and have to rely on information from the live in sister in law who can be unreliable at best with relaying correct information, and a mom that is denial of things. We aren't even sure if he is taking his medication regularly.

My question is, what can we do at this point to get POAs, HIPPA consent forms so we are able to talk to the Dr if he is unable to even sign his own name? He has forgotten how to. We believe he has a lawyer, but we do not know who, and he cannot remember so we do not know if any of these forms have been prepared or not. Any advice for a starting point or possible routes to pursue would be greatly appreciated.
