(utah, Usa) Debt Collection Advice

Card image cap

I got a letter in the mail recently from a collection agency. I am asking for advice on what I can do as I have never had anything to go a debt collection agency before. As far as I was aware, every account had been paid in full.
For details:

The letter they sent me didnt say what the debt was from or who, only that I owed them money for something from 2 years ago. I had a feeling it was an old ambulance bill. The problem is, the letter I received said the date of service was 5/29, and my ambulance bill says 5/30. Not only that, but I spoke with representatives of the ambulance company several times over the phone, a recorded line, and reached a settlement with them to only pay about half of the bill. They set me up on a payment plan, and after I paid the last one, over the phone, I asked the rep if I owed anything else and she said no and that the account was considered paid in full. I stopped receiving correspondence from them (it was only via mail, and then the payment confirmations were email.) Now 2 years later, a collection agency is saying I owe still owe that other half. But even when I did basic math of subtracting what I paid versus the total on the original invoice, the amount the collections agency sent me is off by about 40 dollars. So the total is off and the dates are wrong, and they have records of them telling me its paid in full so...
What can I do? Do I call the agency, the company they are collecting for? Do I talk to a local lawyer? I genuinely have no idea and would appreciate any advice/help.
Thank you

Edit: for an added kicker, the reviews for the agency are terrible with many saying its a scam. Its not BBB accredited, and there are several lawsuits against them. It might be a scam, but Idk for sure.
