Workplace Rescheduled Medical Time Off

My friend scheduled time off two months in advance for a medical appointment and his time off request was approved. Today he was told he cannot take the time off because his manager "needs two days off in a row" and to speak to their district manager. He spoke to them and the matter wasn't settled. The manager who told him he needed to work because they needed two days off in a row now says it's not possible for them to cover because they're now going to be out of town from Sunday through Tuesday, which contradicts their need for two days off in a row and suggests a last minute change in their plan for vacation.

He is wondering if he has any obligation to be there or if there is any legal precedent for him to refuse to come to work on the day he had approved time off for. He is not a part of a union, but a private business that sells sleeping goods.

Any advice would be much appreciated as this situation is causing him lots of stress. Thank you!
