Nursing Home Giving My Loved Ones Someone Else's Meds

Good afternoon, I'm durable POA for my loved one. My loved one has severe dementia and is in a nursing home. Due to the nature of her care I had to move her to a new location

The new location set up a Patient Plan with me and when we went over the medical aspects, it came to light that my LO was being medicated for hypothyroidism, she was being given narcan as needed and she was missing toes.

The good news is that she does physically have all her toes, so I had the new place test her for hypothyroidism since she had never had it before. Turns out she still doesn't have it... So they removed her from the meds.

It turns out that the doctor at the previous location was medicating her unnecessarily for hypothyroidism, no one can figure out the narcan since she has not been on opiates/opioids, and thankfully she does have all her toes.

This feels like a huge malpractice case but I would really like some guidance here, as I've never dealt with anything like this, and I've honestly never thought about suing anyone.

Is this a legit case? I have all docs showing the meds she was on and how the doc at the first place diagnosed her with all these things. What should I, or could I do here?
